Health Office Information and Forms

Pursuant to New York State Education Law Sections 903 and 904, our school district has been selected to report Student Weight Status Category (BMI) information for the 2012-2013 school year. Student Weight Status Category information is collected from the health certificate/appraisal forms submitted during last school year. There is no identifying data, i.e. name, birth date, etc., supplied. If you choose not to have your child’s weight status group information included as part of the Health Department’s survey this year, please print the form below, complete and return this form to your child’s school nurse by December 31, 2012.
